During and after your stay at New York Hospital Queens, caregivers and hospital personnel including, physicians, nurses, technicians, case managers, patient advocates, analysts, etc. may review your medical records. Only personnel, directly involved in patient care, payment and or hospital operations are authorized to access patient information, pursuant to Health Insurance Portability and Accountability Act (HIPAA) guidelines. New York Hospital Queens has taken precautions to insure patient confidentiality at all times.
 
Should you wish to have your information released to another physician, facility, etc. you may sign an Authorization for Release of Health Information Form (click to open). This form is also available through our Health Information Management Department.

You may obtain a form from your nurse or by visiting the department, and someone will assist you with the completion of this form. The HIM Department is open from 8:00 a.m. – 9:00 p.m. and is located on the second floor.  To contact the department from within the hospital call extension 1090 or from outside, call (718) 670-1090.
